2. Download the patient forms, complete them and bring them to your visit

3. Bring payment for your office visit.  The initial visit is $345.  We accept cash only.   Credit cards, checks and other forms of payment are not currently accepted

4. Bring your Florida drivers license or other Florida state issued ID

5. Bring medical documentation from your current treating physician that documents the past work up and treatment of your qualifying medical condition
